Drug allergy discovery

The finding could lead to the development of a diagnostic test to determine drug hypersensitivity.

The study published today in the journal Nature, revealed how some drugs inadvertently target the immune system to alter how the body’s immune system perceives it’s own tissues, making them look foreign.

The immune system then attacks the foreign nature of the tissues as if they were incompatible transplants.

The study showed the biological mechanisms by which a person’s exact tissue type determined whether they would develop the drug allergy or not.

To identify extensor carpi radialis longus (ECRL) motor nerve coordinates in relation to anatomic surface landmarks.

Design

Descriptive study.

Setting

Anatomy institute of a school of medicine in Paris, France.

Cadavers

Fresh adult cadaver upper limbs (N=20).

Intervention

Anatomic dissection of upper limbs.

Main Outcome Measures

Three measurements (mm) were taken of the position of the ECRL motor branch: the distance between the lateral epicondyle and the emergence of the ECRL branch along the forearm axis, the branch depth, and the ratio between the distance corresponding to the nerve depth and the intercondylar distance.

Results

The radial nerves of 4 men and 6 women (age range 59–80y) were identified between the brachioradialis and the ECRL and traced proximally to the lateral epicondyle. The injection point was between the lateral epicondyle posteriorly and the wrist extensor group anteriorly. Direction was perpendicular to the axis of the forearm. Mean depth was 35.6mm (minimal value: 24.0; maximal value: 58.0; SD=9mm). Ratio between nerve depth and intercondylar distance was .46 (.37; .53±.05).

Conclusions

The ECRL motor nerve branch is easy to reach using specified landmarks. Selective motor nerve block of the ECRL branch should be considered in the clinical assessment to test the capacity of the extensor carpi radialis brevis to extend the wrist alone and to assess the command and overactivity of antagonists before surgery.

Fall Attributions among Middle Aged and Older Adults with Multiple Sclerosis

Objectives

(1) To explore the falls attributions of middle aged and older adults with multiple sclerosis (MS), and (2) to examine the personal, health and MS-related factors associated with the three most common attributions.

Design

This was a cross-sectional, descriptive study using data collected through a telephone interview. Falls attributions were obtained through an open-ended question to elicit participants’ stories about their most recent fall.

Setting

United States

Participants

Recruitment was done through a national volunteer MS registry. Three hundred and fifty-four people who were ≥55 years of age were interviewed; 313 provided a falls story. Respondents were primarily married, community-dwelling women who had been living with MS for 21 years, on average.

Intervention

Not applicable

Main outcome measure

The three most common fall attributions were used as dependent variables to address the second research objective.

Results

A total of 14 falls attributions were identified. The most common were balance (41.5%), lower extremity malfunction (31.0%) and assistive technology (AT) (29.7%). Falls control was significantly associated with the balance attribution (OR = 0.51, CI = 0.29-0.88), no variables were associated with lower extremity malfunction attribution, and use of multiple mobility devices was significantly associated with the AT attribution (OR = 3.78, 95% CI = 2.09-6.85)

Conclusion

Findings highlight the complex nature of falls among middle-aged and older adults with MS and point to the need for comprehensive fall prevention interventions for this population. Further investigation of the role that perceived control over falls plays in this population is warranted.

To examine the velocity-dependent change in medial gastrocnemius (MG) activity during the stance phase of gait in patients with moderate to severe resting hypertonia after stroke or traumatic brain injury (TBI).

Design

Cohort study.

Setting

Motion analysis laboratory in a tertiary-care rehabilitation hospital.

Participants

Convenience sample of patients with chronic TBI and stroke (n=11 each), and age- and sex-matched healthy controls (n=22).

Intervention

Not applicable.

Main Outcome Measures

Frequency and gain (steepness) of positive (>0) and significant positive (>0 and goodness of fit P≤.05) electromyogram–lengthening velocity (EMG-LV) linear regression slope in MG during the stance phase of gait.

Results

Positive and significant positive slopes were found significantly more often on the more affected (MA) than less affected (LA) side in patients with TBI but not stroke. Both the frequencies of positive and significant positive slopes on the MA side in patients with TBI were also significantly higher than in controls. However, neither the gain of positive nor significant positive EMG-LV slope was different between the MA and LA sides or in comparison with controls. Positive slope parameters were not related to Ashworth score on the MA side.

Conclusions

The frequency and gain of positive EMG-lengthening slope did not effectively differentiate patients from controls, nor were they related to the resting muscle hypertonia. Motor output during MG lengthening in the stance phase of gait is apparently not exaggerated or related to resting hypertonia in patients with chronic TBI and stroke. Thus, changes in gait during stance cannot be ascribed to increased stretch reflex activity in MG muscle after acquired brain injury.

Improved Clinical Status, Quality of Life and Walking Capacity in Parkinson’s Disease after Body-Weight Supported High-Intensity Locomotor Training

Objective

To evaluate the effect of body-weight supported progressive high-intensity locomotor training in Parkinson’s disease (PD) on: (1) clinical status; (2) quality of life; and (3) gait capacity.

Design

An open-label, fixed sequence cross-over study.

Setting

University motor control laboratory.

Participants

Thirteen patients with idiopathic PD (Hoehn & Yahr 2.1) and stable medication use.

Interventions

Patients completed an eight-week (3x1hour/week) training program on a lower-body positive-pressure treadmill. Body-weight support was used to facilitate increased intensity and motor challenges during treadmill training. The training program contained combinations of 1) running and walking intervals, 2) the use of sudden changes (e.g. in body-weight support and speed), 3) different types of locomotion (e.g. chassé, skipping and jumps) and 4) sprints at 50 % body-weight.

Main outcome measures

The Movement Disorders Society Unified-Parkinson’s-Disease-Rating Scale (MDS-UPDRS), Parkinson’s-Disease Questionnaire 39 (PDQ-39) and the six-minute walk test, eight weeks before, pre and post training.

Results

At the end of training, statistically significant improvements were found in all outcome measures compared to the control period. Total MDS-UPDRS changed from (mean±1SD) 58±18 to 47±18, MDS-UPDRS motor part from 35±10 to 29±12, PDQ-39 summary index from 22±13 to 13±12 and the six-minute walking distance from 576±93m to 637±90m.

Conclusions

Body-weight supported progressive high-intensity locomotor training is feasible and well tolerated by patients with PD. The training improved clinical status, quality of life and gait capacity significantly.

St Jude device lowers blood pressure in small study

St Jude Medical Inc said on Monday its procedure that deadens nerves near the kidneys helped lower blood pressure in a small study of patients whose hypertension could not be controlled with drugs.

Patients in the study who were treated with the Enlightn renal denervation system saw an average reduction of 28 points in systolic blood pressure, which is the first number expressed in a reading, after 30 days. At six months, the 46 patients who received the treatment maintained an average systolic blood pressure reduction of 26 mmHg points.

The results were presented at the annual meeting of the American Heart Association in Los Angeles.

Patients enrolled in the study had an average blood pressure of 176/96 mmHg despite being treated with multiple medications to manage the condition. No serious side effects were reported, St Jude said.

Why astronauts experience low blood pressure after returning to Earth from space

When astronauts return to Earth, their altitude isn’t the only thing that drops—their blood pressure does too. This condition, known as orthostatic hypotension, occurs in up to half of those astronauts on short-term missions (two weeks or less) and in nearly all astronauts after long-term missions (four to six months). A new research report published online in The FASEB Journal (http://www.fasebj.org) solves the biological mystery of how this happens by showing that low gravity compromises the ability of arteries and veins to constrict normally, inhibiting the proper flow of blood. Prevention and treatment strategies developed for astronauts may also hold promise for elderly populations on Earth who experience orthostatic hypotension more than any other age group.

“The idea of space exploration has been tantalizing the imagination of humans since our early existence. As a scientist, I have had the opportunity to learn that there are many medical challenges associated with travel in a weightless environment, such as orthostatic hypotension, bone loss and the recently recognized visual impairment that occurs in astronauts,” said Michael D. Delp, Ph.D., a researcher involved in the work from the Department of Applied Physiology and Kinesiology, and the Center for Exercise Science at the University of Florida in Gainesville, Florida. “Although I have come to realize that it is unlikely I will ever get to fulfill my childhood dream of flying in space, I take great satisfaction with helping in the discovery of how microgravity alters the human body and how we can minimize these effects, so humans can safely explore the bounds of our universe.”

To make this discovery, Delp and colleagues examined arteries and veins from mice housed at Kennedy Space Center in Florida with blood vessels from groups of mice flown on three of the last five space shuttle missions - STS-131, STS-133 and STS-135. Mice flown on the STS-131 and STS-135 missions were tested immediately after returning to Earth, whereas mice from STS-133 were tested one, five and seven days after landing. Not only did they find that these mice experienced the equivalent of orthostatic hypotension in humans, they also discovered that it takes as many as four days in normal gravity before the condition is reversed.

Hypertension not so simple

A recently published editorial in the Journal of the American Society of Hypertension (JASH), “Ambulatory Blood Pressure Monitoring Should Be Included in the National Health and Nutritional Examination Survey (NHANES),” recognizes the importance of this national survey instrument but questions the efficiency of its diagnostic methods in assessing hypertension in the population.*

Since the 1960s, CDC has utilized traditional blood pressure screening using a sphygmomanometer to measure the brachial artery pressure (a diagnostic instrument used since 1880). Drs. William B. White, President of the American Society of Hypertension, Inc. (ASH), Thomas D. Giles, a Past President of ASH, and colleagues have expressed concern regarding this methodology and its limitations in accurately detecting hypertension in patients. Their editorial strongly recommends that the CDC utilize a more sensitive and specific tool for assessing blood pressure in its national surveys.

Their recommendation for improving national statistics on hypertension is the inclusion of ambulatory BP monitoring in the NHANES’ methodology. Among many physicians and hypertension specialists, ambulatory BP monitoring has been recognized as a superior and accurate method for detecting hypertension. The utilization of this advanced diagnostic method may bring to light many mitigating factors that affect the proper diagnosis of this condition, such as white coat hypertension, resistant hypertension, and nocturnal hypertension. Leading hypertension specialists around the country recognize that ABPM can not only detect hypertension in cases in which the disease actually exists, but also identify who is really in control and who is not.

The tropical island of Mauritius, located off the coast of Southern Africa, has greatly improved its health care system, especially for frontline services and procedures such as cardiac surgery. But the post–acute neurorehabilitation care is still problematic, much to the detriment of disabled patients, their families, and Mauritian society overall. Comparisons with neurorehabilitation care in the United Kingdom suggest the scale of the problem in terms of uncoordinated medical teams, limited follow-ups, lack of expertise, and cultural stigma. This article assesses the needs of the neurologic rehabilitation segment in Mauritian health care and submits a set of policy recommendations addressing what medical professionals, hospitals, government officials, and other organizations can do to improve the neurologic rehabilitation infrastructure for Mauritian patients.

Objectives

To assess the reproducibility and validity of the 6-minute walk test (6MWT) with gas collection, and to evaluate effectiveness of a walking program in children with cerebral palsy (CP).

Design

Assessment and controlled training study.

Setting

Rehabilitation service.

Participants

Children/adolescents with CP (N=24; 12 boys, 12 girls; mean age, 14.2±2.0y, Gross Motor Function Classification System levels I and II).

Intervention

After a cycle-ergometer stress test and the 6MWT, subjects were assigned to training (n=12, 40min of moderate walking exercise 3 times per week for 8wk) or a matched control group (n=12).

Main Outcome Measures

Peak oxygen consumption (V̇o 2peak), peak ventilation, peak heart rate, and 6MWT distance.

Results

Test-retest correlations for the 6MWT were good (V̇o 2peak: r=.90, P<.001, intraclass correlation coefficient [ICC]=.85; peak ventilation: r=.88, P<.001, ICC=.83; peak heart rate: r=.86, P<.001, ICC=.82; distance walked: r=.87, P=.007, ICC=.80). Mean scores for the 6MWT also closely matched corresponding cycle-ergometer data. Significant improvements in 6MWT V̇o 2peak, peak ventilation, and peak heart rate were found after 8 weeks of training (P<.05).

Conclusions

The 6MWT appears reproducible and valid relative to cycle-ergometer assessments of cardiorespiratory responses, and offers a simple method of clinical assessment. An 8-week moderate walking program improves the cardiopulmonary fitness of children with CP, as measured by 6MWT.

Protein reveals diabetes risk many years in advance

When a patient is diagnosed with type 2 diabetes, the disease has usually already progressed over several years and damage to areas such as blood vessels and eyes has already taken place. To find a test that indicates who is at risk at an early stage would be valuable, as it would enable preventive treatment to be put in place.

Researchers at Lund University have now identified a promising candidate for a test of this kind. The findings have been published in the journal Cell Metabolism.

“We have shown that individuals who have above-average levels of a protein called SFRP4 in the blood are five times more likely to develop diabetes in the next few years than those with below-average levels”, says Anders Rosengren, a researcher at the Lund University Diabetes Centre (LUDC), who has led the work on the risk marker.

Higher levels in diabetes patients

It is the first time a link has been established between the protein SFRP4, which plays a role in inflammatory processes in the body, and the risk of type 2 diabetes. Studies at LUDC, in which donated insulin-producing beta cells from diabetic individuals and non-diabetic individuals have been compared, show that cells from diabetics have significantly higher levels of the protein.

To compare the ankle muscle strength and torque-angle relationship between individuals with Parkinson's disease (PD) and participants without impairments.

Design

Cross-sectional, exploratory study.

Setting

Motor control laboratory in a university.

Participants

Convenience sample of community-dwelling individuals with PD (n=59) recruited from a PD self-help group and age-matched participants without impairments (n=37) recruited from community older adult centers.

Interventions

Main Outcome Measure

Peak torque and angle-torque profile during concentric and eccentric contraction of ankle dorsiflexors and plantarflexors at 2 different angular speeds (45 and 90°/s).

Results

The PD group displayed lower muscle peak torque values than participants without impairments in all test conditions. Generally, concentric strength was more compromised, with a greater between-group difference (Cohen d=1.29–1.60) than eccentric strength (Cohen d=.81–1.37). Significant group by angular speed interaction was observed in ankle plantarflexion concentric peak torque (P<.001), indicating that muscle weakness was more pronounced when the angular speed was increased. The group by joint angle interaction in concentric contraction of ankle plantarflexors at 90°/s was also significant (P<.001), revealing that the between-group difference in torque values became increasingly more pronounced when the joint was moving toward the end range of the ankle plantarflexion. This exaggerated ankle plantarflexor muscle weakness at the end range was significantly correlated with clinical balance measures (P<.05).

Conclusions

Muscle weakness in PD is influenced by contraction type, angular speed, and joint range. Exaggerated weakness is found in concentric contraction of ankle plantarflexors, particularly when the angular speed is high and the muscle is in shortened lengths.

Objective

To study the relationship between balance confidence, a concept closely related to fear of falling, mobility and balance performance, and perceived mobility limitation in older people after a fall-related hip fracture.

Design

Cross-sectional analyses of pretrial data of 2 randomized controlled trials of physical rehabilitation.

Setting

University research center.

Participants

Community-dwelling people aged over 60 years, 6 weeks to 7.5 years after a fall-related hip fracture (N=130).

Interventions

Main Outcome Measures

The main outcome was the self-reported Activities-specific Balance Confidence (ABC) scale score. Assessments also included perceived ability to walk outdoors or climb 1 flight of stairs, and assessments of self-preferred walking speed, modified Timed-Up-and-Go test, and Berg Balance Scale.

Results

Higher ABC scale scores were related to better mobility and balance performance (ρ>.47) and perceived mobility function (ρ>.54). In univariate general linear models, all associations also remained significant after adjustment for age, sex, time since fracture, number of chronic diseases, and either level of physical activity or muscle strength of the fractured leg. An ABC scale score <85 points identified those with mobility and balance limitation across measures.

Conclusions

In people who have had a fall-related hip fracture, an independent relationship exists between balance confidence and mobility and balance performance as well as perceived mobility function. Since lack of balance confidence may compromise rehabilitation and recovery, the ABC scale may help to identify older hip fracture patients with mobility and balance limitation.
